Living Will

In this document, you make your wishes known as to whether you want a DNR (do not resuscitate order) and whether you desire to forego any life-sustaining treatment to prolong life, including cardiopulmonary resuscitation (CPR) and artificially or technically supplied hydration and nutrition if you are in one of two conditions: (i) you are in a permanently unconscious state (i.e., an irreversible condition in which you are unable to feel pain and have lost higher brain function, where you are permanently unaware of yourself and surroundings, and cannot feel pain) or (ii) you have an irreversible, incurable, and untreatable terminal condition where there can be no recovery and death is imminent without the life-sustaining treatment.   The agent that you appoint in your Living Will is an individual that you will appoint to ensure your wishes are carried-out in these two limited situations.
